Pesanan untuk Penuntut perubatan

1. Adalah perlu bagi penuntut ilmu perubatan belajar dengan penuh tanggungjawab sehingga dia menjadi pakar dalam bidangnya. Ketika ayahku meninggal ketika aku masih belum tamat pengajian perubatan, aku terfikir untuk keluar bekerja dan mengumpul duit. Syaitan sentiasa mendorong berbuat demikian namun aku mengambil keputusan untut tetap meneruskan pengajian walaupun dalam keadaan payah.

Imam Syafie berkata:” Jika kamu meninggalkan seorang guru ketika belum sampai masanya, maka Allah akan haramkan kebaikan padamu. Menuntut ilmu memerlukan masa yang panjang dan kecerdikan (zakiyy)”.

2. Seorang doktor Muslim perlu menguasai ilmu qawaid fiqhiyyah dan qawaid tibbiyah. Perlu juga menguasai maqasid syari’yyah iaitu menjaga akal, harta, agama, jiwa dan nasab. Contohnya dalam masalah percampuran nasab seperti bank sperma, ini dilakukan oleh mereka yang jahil dan tak beriman kepada Allah. Tak patut bagi seorang doktor Muslim melakukan demikian kerana kita adalah kaum yang dimuliakan Allah dengan Islam.

3. Seorang guru kamu yang mahir dalam ilmu perubatan umpama syeikh kamu dalam agama. Bersungguh-sungguhlah menuntut ilmu daripadanya.

4. Doktor Muslim perlu menguasai sepenuhnya ilmu perubatan. Jika dia mengambil keputusan untuk membedah seorang pesakit, padahal ada cara lain yang lebih baik untuk merawat pesakit tersebut, tetapi tidak diketahui oleh doktor tersebut lantaran kurang ilmunya, maka dia akan disoal di hadapan Allah di akhirat nanti. Doktor perlu tahu sejauh mana keperluan untuk melakukan pembedahan atau tidak, terutama kepada pesakit yang datang yang minta dibedah untuk mengubah ciptaan Allah. (pembedahan plastik untuk tujuan kecantikan). Doktor Muslim tersebut perlu mengetahui hukum dan menasihati pesakit tersebut dengan baik.

5. Doktor Muslim perlu menghormati manusia sebagai ciptaan Allah. Sebenarnya, kebanyakan ilmu perubatan yang dipelajari hari ini diambil dari fahaman Barat yang menganggap manusia umpama sekeping daging yang tak berguna, bukan makhluq Allah yang perlu dihormati. Kita perlu perbetulkan kembali fahaman yang salah ini. Hubungan dengan pesakit adalah hubungan kasih sayang dan rahmah dengan makhluq Allah, bukan merawat dengan tujuan untuk membuat uji kaji atau bereksperimen. Anggaplah pesakit umpama diri kita sendiri yang perlu ditolong, pasti kita akan memberikan rawatan yang terbaik.

6. Doktor Muslim perlu arif dengan perkembangan terkini dan semasa ilmu perubatan, kerana ilmu ini berkembang dengan pesat, serta kaitkannya dengan hukum agama.

7. Doktor wajib menjaga rahsia dan aurat pesakit, dilarang berdua-duaan dengan pesakit yang berlainan jantina. Buka aurat untuk pemeriksaan sekadar perlu @ darurat @ jika tak dapat dielakkan sahaja.

8. Pastikan doktor Muslim ini berbeza dengan doktor bukan Islam. Kamu bukan dijadikan untuk dunia, tetapi kamu dijadikan untuk akhirat. Rezeki sudah ditentukan Allah, itqan (bersungguh-sungguh) dalam pekerjaanmu itulah yang dituntut. Hikam Ibhu A’thaillah menyebut: kamu sibuk dengan perkara yang telah ditentukan (rezeki), dan menjauhkan diri dari perkara yang dituntut (bersungguh-sungguh) adalah dalil Allah mencabut pandangan baik dari jiwa kamu.

9. Saidina Ali menyebut: ” Kedudukan seseorang adalah pada apa yang dia kuasai, bukan apa yang dia miliki. Apa yang mampu kita beri pada orang lain itu lebih baik daripada apa yang kita miliki”.

10. Penting bagi seorang doktor Muslim terlibat dengan misi kemanusiaan. Aku pernah bertanya kepada Dr. Abdul Qadir Qutb, seorang pakar pembedahan tentang bagaimana untuk menjadi hebat dalam bidang ini? Beliau menjawab: Mahu tak mahu, kamu perlu terlibat dalam misi peperangan, maka kamu akan mendapat pengalaman umpama kamu terlibat dalam ilmu perubatan selama 20 tahun.

11. Ketika terpilih untuk misi kemanusiaan di sempadan Afghanistan, aku sempat merawat 150 orang pesakit. Seorang doctor Afghanistan merasa kagum lantaran banyak pembedahan yang aku lakukan berjaya. Beliau bertanya apakah rahsianya dan aku menjawab; Aku bukan membuatnya dengan tangan, tetapi aku membuatnya dengan hati.

12. Pesanan untuk pelajar perubatan; tuntutlah ilmu untuk mencari redha Allah, bukan tujuan untuk dipandang manusia. Ilmu perubatan adalah ilmu yang bermanfaat untuk manusia. Tanda menuntut ilmu kerana Allah: panjang masanya, bersungguh-sungguh, rendah diri kepada sahabat2 dan pesakit, hormati guru yang mengajar kita. Kekallah dengan apa yang telah ditentukan Allah, itu adalah tanda wali Allah.

~Manusia adalah ciptaan Allah yang agung, jadi persediaan untuk merawat makhluq Allah ini perlulah hebat.

Untuk ke sekian kalinya, saya ditegur kerana tidak mengemaskinikan blog. Mak pun sudah pesan, tak boleh berhenti, walau sibuk mana pun. Mungkin, belakangan ini saya gagal menguruskan masa dengan baik. Urusan walimah adik perempuan saya, urusan peperiksaan di tempat kerja, urusan penjadualan HO di wad, urusan peribadi yang lainnya… Oh, banyak alasan. Walimah dah berlalu, peperiksaan dah lulus, dan jawatan kerja tambahan sebagai ketua pengurus HO di wad sudah diberikan kepada orang lain. Tapi masih gagal menulis. Mungkin kerana semakin kurang berfikir, semakin kurang membaca (walaupun buku berak-rak terbeli sampai dah roboh, tapi tak sampai separuh yang dah dibaca). Usrah juga sudah beberapa kali tertinggal. Oh, kalau zaman kampus dulu pasti sudah diberi amaran: Futur! +_+

“…This Angel is commanded to write Four decrees: that he writes down his provision (rizq), his life span, his deeds, and whether he will be among the wretched or the blessed…” (potongan hadith ke-4 dalam Hadith 40 an-Nawawi)

Allah mengetahui segala sesuatu dengan ilmu-Nya yang azali.

Saya masih ingat, Shaykh Riad mengatakan sifat al-Qadir dan al-Muqtadir Allah berkait dengan sifat Allah al-‘Alim. Allah Maha mengetahui segala keadaan makhluk-Nya yang berupa ketaatan, rezeki mahupun ajal. Dan pengetahuan Allah ini bersifat azali, sebelum pun qadar atau ketetapan Allah ini dituliskan di dalam ummul-KItaab yakni Luh Mahfuzh 50 ribu tahun sebelum diciptakan langit dan bumi. [Rujuk Sahih Muslim – hadith #6416]

“Do you not know that Allah knows what is in the heaven and earth? Indeed, that is in a Record (Luh Mahfuzh). Indeed that, for Allah , is easy.” [Al-Hajj: 70]

Verily, the first of what Allah created was the Pen, and He said to it: “Write.” The Pen said: “O my Lord, what shall I write” He said: “Write the decree and whatever will throughout eternity.” [recorded by Imam Ahmad through various routes of transmission. At-Tirmidhi also recorded it from a Hadith of Abu Dawud and he (At-Tirmidhi) said about it, ‘Hasan Sahih, Gharib.’]

Empat kalimah yang ditulis di peringkat janin

Malaikat diutuskan kepada janin untuk ditiupkan ruh selepas 40 hari, dan pada waktu itu juga dituliskan 4 perkara/ketentuan ke atas janin tersebut: Rezekinya, umurnya, amalnya dan sama ada dia termasuk dalam golongan yang bahagia atau celaka. (Rujuk Hadith ke-4 dalam Hadith 40 an-Nawawi)

Dan terdapat banyak hadith lain yang memperincikan setiap satu daripada 4 perkara tersebut. (Sahih Muslim, kitab al-Qadr)

‘Abdullah b. Mas’ud reported: Evil one is he who is evil in the womb of his mother and the good one is he who takes lesson from the (fate of) others. The narrator came to a person from amongst the Companion of Allah’s Me ssenger (may peace be upon him) who was called Hudhaifa b. Usaid Ghifari and said: How can a person be an evil one without (committing an evil) deed? Thereupon the person said to him: You are surprised at this, whereas I have heard Allah’s Messenger (may peace be upon him) as saving: When forty nights pass after the semen gets into the womb, Allah sends the angel and gives him the shape. Then he creates his sense of hearing, sense of sight, his skin, his flesh, his bones, and then says: My Lord, would he be male or female ? And your Lord decides as He desires and the angel then puts down that also and then says: My Lord, what about his age? And your Lord decides as He likes it and the angel puts it down. Then he says: My Lord, what about his livelihood? And then the Lord decides as He likes and the angel writes it down, and then the angel gets out with his scroll of destiny in his hand and nothing is added to it and nothing is subtracted from it.

Kebetulan, atau memang Allah mahu saya belajar tentangnya, banyak pula syndromic babies/children yang dimasukkan ke wad kanak-kanak sejak akhir-akhir ni. Kalau masa student dulu, mungkin teruja bila dapat jumpa kes yang macam-macam. Membaca kurang nak masuk ke otak, melihat sendiri, lebih mudah ingat. Tapi dalam realiti pekerjaan, menjadi doktor bukan sekadar merawat penyakit, tetapi termasuk mengendalikan pesakit dan keluarganya sebagai seorang individu. Iya, orang putih kata holistic approach.

Dan hadith ini, sedikit sebanyak menyentuh hati saya:

Abu Tufail reported: I visited Abu Sariha Hudhaifa b. Usaid al-Ghifari who said: I listened with these two ears of mine Allahs Messenger (may peace be upon him) as saying: The semen stays in the womb for forty nights, then the angel, gives it a shape. Zubair said: I think that he said: One who fashions that and decides whether he would be male or female. Then he (the angel) says: Would his limbs be full or imperfect? And then the Lord makes thein full and perfect or otherwise as He desires. Then he says: My Lord, what about his livelihood, and his death and what about his disposition? And then the Lord decides about his misfortune and fortune.

Benarlah Allah telah menentukan semuanya ke atas manusia. Kadang-kadang manusia terlupa bahawa pengetahuan kita sangat terbatas, tak terjangkau pengetahuan Allah ‘azza wa Jalla. Berada dalam bidang sains ini, kita hanya melihat setiap proses yang berlaku itu hanyalah sekadar proses yang spontan – disebabkan kecacatan ‘gene’ dsb, walhal setiap satu daripadanya tidak terlepas daripada pengawasan Allah. Kerap kali saya melihat ibu-bapa mempersoalkan, mengapa anak mereka begitu dan begini, masing-masing mencari punca, hatta ada yang menyalahkan satu sama lain hingga berlaku perceraian. Semoga Allah jauhkan.

Namun banyak juga yang saya pelajari daripada ketabahan dan kesabaran ibu bapa yang anak mereka tidak sempurna. Saya percaya, sabar dan redha itu hanya mampu terbentuk apabila tawakkal dan pergantungan mereka kepada Allah sentiasa subur. Duhai ibu-ibu, semoga Tuhan merahmati-Mu dan menggantikan yang lebih baik untukmu disebabkan kesabaranmu!

…I swear by Allah – there is no God but He – one of you may perform the deeds of the people of Paradise till there is naught but an arm’s length between him and it, when that which has been written will outstrip him so that he performs the deeds of the people of the Hell Fire; one of you may perform the deeds of the people of the Hell Fire, till there is naught but an arm’s length between him and it, when that which has been written will overtake him so that he performs the deeds of the people of Paradise and enters therein.” (potongan hadith ke-4 dalam Hadith 40 an-Nawawi)

… Dan di hujung usia…

Tidak ada sesuatu jiwa yang dilahirkan melainkan Allah telah menulis tempatnya di dalam syurga atau neraka. Telah ditulis juga sama ada ia akan celaka atau bahagia. Lalu seorang lelaki bertanya, “Wahai Rasulullah, kenapa kita tidak berserah kepada kitab (taqdir) kita dan meninggalkan amal?” Baginda bersabda, “Beramallah, kerana setiap orang dipermudahkan untuk melakukan apa yang diciptakan untuknya. Golongan bahagia dipermudahkan untuk melakukan amalan orang bahagia, manakala golongan celaka dipermudahkan untuk melakukan amalan golongan celaka,” lalu baginda membaca ayat:

As for he who gives and fears Allah And believes in the best [reward], We will ease him toward ease. But as for he who withholds and considers himself free of need And denies the best [reward], We will ease him toward difficulty. (Al-Layl: 5-10) [Riwayat Bukhari (1362) dan Muslim (2647)]

Imam Ahmad rahimahuLLah meriwayatkan daripada Anas b Maalik:

Janganlah kamu berasa kagum dengan seseorang sehingga kamu melihat bagaimana kesudahannya, kerana sesungguhnhya seseorang itu beramal dalam tempoh yang panjang daripada umurnya atau masanya, dengan amalan yang baik. Jika ia mati dalam keadaan itu, dia akan masuk syurga. Namun kemudiannya dia berubah dan melakukan amalan jahat. Sesungguhnya seseorang itu pula beramal dalam tempoh yang panjang dengan amalan jahat. Jika ia mati dalam keadaan itu, dia akan masuk neraka. Namun kemudiannya ia berubah, lalu melakukan amalan yang baik.” [Riwayat Ahmad 3/120 – isnadnya sahih]

Imam ibn Rajab mengulas, pada pandangan manusia, hakikat sebenar kelihatan berbeza dengan yang zahir. Sedang kesudahan yang buruk itu disebabkan oleh rahsia batin yang tersembunyi daripada pandangan manusia, sama ada berbentuk amalan jahat dsb, maka sifat yang tersembunyi itu yang menyebabkan kesudahan buruk ketika mati. Semoga Allah hindarkan. [Jaami’ Uloom wal-hikaam]

Kesimpulannya, ‘Don’t judge a book by its cover’. Dan jangan sesekali berputus asa dengan pelaku dosa serta maksiat, kerana bukan kita yang menilai dan menentukan kesudahan mereka (dan kita), sebaliknya ALLAH lebih mengetahui tempat tinggal kekal mereka sesudah mereka (dan kita) dibangkitkan daripada kematian.

A 14-year old girl was brought in to the resuscitation bay in emergency depeartment yesterday night, with no sign of life. Cardiac monitoring showed asystole and CPR was commenced immediately. She was revived, but then went into VT and asystole. Another cycle of CPR was commenced, again, she was revived but the same sequence continues. The child was eventually pronounced dead after 90 minutes of resuscitation.

A heart-breaking news for the parents indeed, and for us as well. Should the child was brought earlier to the hospital, she might be saved. The cause of death was unknown, however from the history obtained from the parents – altered behaviour, blank stare, abnormal limbs movement, lethargy – likely cause is meningoencephalitis: an acute inflammation of brain parenchyma as well as the membranes surrounding the brain.

The mother was in denial, hardly accepting the bad news, as her child was previously healthy with no co-morbidity, a bright student and active in school. And her greatest regret is to believe that child was possessed by some spirit (jinn) as told by the ‘bomoh’ they visited few days back, at the initial onset of illness. As a result, child was kept at home despite being unwell for many days and was only brought to hospital once child was very ill, cyanosed (blue) and unresponsive.

It is disappointing indeed, to know how some people react to certain illnesses. It is common in our culture, to seek help from the ‘bomoh’ or ‘dukun’ rather than going to the clinic or hospital to get further treatment. I am not against that though, as long as the principle of healing technique is parallel to what our prophet had taught us. Everyone has the right to seek treatment alternative to what is being offered in hospital. However, what triggers my concern is, many people still have stigma on certain illnesses especially psychiatric illness as well as TB, epilepsy and leukaemia, just to name few. They rather be labeled as being possessed by the spirit or was sent some black magic than being labeled with such illnesses. It is not that I don’t believe in the world of unseen, the black magic and all, but can’t we be sensible enough to think when to seek medical attention in hospital? And why would we easily believe or choose to believe that other people sent us black magic out of envy and grudge? Are we that great to think that we are as noble as angels and others are as evil as demons?

O you who have believed, if there comes to you a disobedient one with information, investigate, lest you harm a people out of ignorance and become, over what you have done, regretful. [Hujurat: 6]

O you who have believed, avoid much [negative] assumption. Indeed, some assumption is sin. And do not spy or backbite each other. Would one of you like to eat the flesh of his brother when dead? You would detest it. And fear Allah ; indeed, Allah is Accepting of repentance and Merciful. [Hujurat: 12]

As for this unfortunate girl’s case, the death is preventable. I mean, death due to this illness is preventable if intervention  is started earlier. But I didn’t mean that doctor’s efforts may overrule Allah’s Qadr upon her. Of course, our belief as a Muslim, if it has pre-ordained upon her to die on the same day at the same time, nothing can stop it from happening. Doctors are just mere human, and Allah is the Most Able and Powerful. But at least, if something was done earlier, perhaps we won’t have so much of regret if the child still fail to survive out of her illness.

Usamah ibn Shuraik narrated: “I came to the Prophet, peace be upon him, and found him with his companions. They were calm and serene as if there were birds over their heads. I greeted them and sat down. Then some bedouins came from various places. They asked him: ‘O Allah’s Messenger! Should we seek medical treatment for our illnesses?’ He replied: ‘Yes, you should seek medical treatment, because Allah, the Exalted, has let no disease exist without providing for its cure, except for one ailment, namely, old age’.” (Reported by Ahmad and the six sunan. Tirmidhi considers it a sound hadith.)

Anas ibn Mas’ud reported that the Prophet, peace be upon him, said: “Verily, Allah has not let any malady occur without providing its remedy. Therefore seek medical treatment for your illnesses.” (Nasa’i, Ibn Majah, and al-Hakim.)

Jabir narrated that the Messenger of Allah, peace be upon him, said: “There is a cure for every disease. Whenever an illness is treated with its right remedy, it will, by Allah’s permission, be cured.” (Muslim)

Alhamdulillah, saya telah tamat ‘tagging‘ untuk posting ke-5 saya di NICU. (Neonatal intensive care unit). Dan pagi sabtu lalu, saya dikhabarkan dengan kelahiran anak kedua sahabat saya. Alhamdulillah, alhamdulillah, alhamdulillah. Saya sangat gembira. Mungkin bagi orang lain, kelahiran merupakan satu proses yang biasa, tapi bagi saya, ia merupakan satu keajaiban Allah yang cukup besar. Barangkali ada yang hairan, saya yang berada di aliran perubatan berfikir demikian. Setiap hari berhadapan dengan 1001 pesakit, membuatkan saya insaf dan bersyukur atas segala kurniaan-Nya dari sekecil-kecil nikmat.

Kisah di NICU dan IVF

Saya merenung bayi sekecil 2 telapak tangan saya. Pernafasannya masih bergantung kepada ‘ventilator’ akibat paru-paru yang tidak matang (chronic lungs disease / bronchopulmunory dysplasia) meskipun ubat untuk mematangkan paru-paru sudah diberikan. Kasihan anak ini. Tetapi lebih derita kedua ibu bapanya. Mereka berikhtiar dengan IVF (In-vitro fertilization / kaedah persenyawaan beradas) demi mendapatkan seorang cahaya mata. Bukan mudah, bukan murah. Saya terfikir, kalau tak berduit, bagaimana ya? Wahai, bagi wanita yang mudah hamil dan mudah pula bersalin, tunduk dan sujudlah kepada TuhanMu!

Dan IVF itu berjaya, si ibu disahkan mengandung kembar tiga. (Kehamilan melalui kaedah ini mempunyai kebarangkalian yang tinggi untuk menghasilkan janin kembar) Dan kehamilan kembar kebiasaannya mempunyai risiko-risiko tersendiri kepada ibu dan juga bayi. Dan risiko terbesar adalah melahirkan bayi dalam keadaan pra-matang (sebelum 37 minggu). Dan bayi pra-matang juga mempunyai komplikasi-komplikasi yang tersendiri. Semakin awal kelahiran (sangat pra-matang), semakin tinggi risiko untuk mengalami komplikasi.

Ubun-ubun bayi itu saya usap. Duhai anak, lekas-lekas sembuh, kasihan ibu-bapa mu, bersusah payah untuk mendapatkanmu. Dan setelah kau lahir, mereka masih diuji dengan keadaanmu yang kritikal ini. Semoga kau membesar menjadi anak yang soleh, jambatan untuk menyampaikan hidayah Allah kepada kedua ibu bapamu. Ya, bayi ini anak kepada pasangan suami-isteri bukan islam. Tetapi anak ini masih fitrah. Saya tidak tahu bagaimana harus saya mendoakan anak ini.

اِذْهَبِ البَأْسَ رَبَّ النَّاسِ وَاشْفِ أَنْتَ الشَّافِي لاَ شِفَاءً إِلَّا شِفَاؤُكَ شِفَاءً لَا يُغَادِرُ سَقَماً
Jauhkanlah dia dari kesakitan, wahai Tuhan sekalian manusia, sembuhkanlah dia, dan engkaulah maha penyembuh yang tidak akan datang kesembuhan kecuali datang dariMu, kesembuhan dari jenis yang tidak akan kembali penyakitnya” [Riwayat Muslim]

ربِّ اجْعَلنِي مُقِيمَ الصَّلاةِ وّمِنْ ذُرِّيَّتِي رَبَّنّا وَتَقَبَّلْ دُعَاء (40) رَبَّنَا اغْفِرْ لِيْ وَلِوَالِدَيَّ وَلِلمُؤمِنِينَ يَوْمَ يَقُومُ الحِسَاب (41
“My Lord, make me an establisher of prayer, and [many] from my descendants. Our Lord, and accept my supplication. Our Lord, forgive me and my parents and the believers the Day the account is established. [Ibrahim: 40-41]


Kisah seorang sahabat & Premature ovarian failure

Mak, kawan kakak F dah pregnant!” Saya mengkhabarkan kepada ibu saya berkenaan teman sekuliah saya dulu. Excited.
Dah kahwin, biasalah tu pregnant. Apa yang nak dihairankan?” Saya terdiam. Tapi saya tak tahu kenapa, sejak dulu, setiap kali saya mendapat khabar bahawa teman saya hamil, pasti saya ingin kongsikan kegembiraan itu. Kehamilan bukan sekadar rutin bagi seorang isteri/ibu, tetapi satu anugerah. Mungkin bagi wanita yang mudah conceive, ia melihatnya sebagai rutin. Tetapi bagi yang perlu berusaha lebih untuk hamil, hanya Allah yang lebih mengetahui kegembiraan si ibu dan bapa bayi tersebut.

Dan bayi hasil IVF di atas mengingatkan saya kepada sahabat yang baru melahirkan anak Sabtu lalu. Bagi saya ia merupakan satu keajaiban yang besar. Beberapa bulan sebelum pernikahannya, doktor mendiagnosa bahawa dia menghidap ‘premature ovarian failure‘ iaitu kemerosotan atau kegagalan fungsi ovari (organ yang menghasilkan telur/ovum) sebelum umur 40. Pesakit tersebut akan mengalami simptom-simptom menopaus sebelum umur 40; haid yang tidak teratur dan jarang-jarang. Dan sebahagian besar pesakit tidak akan mampu hamil melainkan dengan rawatan kesuburan (fertility treatment) dan assisted conception seperti Intrauterine insemination dan IVF. Dan bagi kami di aliran perubatan, ia merupakan satu keajaiban jika wanita tersebut mampu hamil tanpa assisted conception. Dan subhanaLLah, ini adalah kelahiran anaknya yang kedua!

Ya, Allah berada pada sangkaan KITA hambaNya.

Saya yakin saya akan mengandung, sebab saya dah berdoa kepada Allah.”

Kalau dulu-dulu, saya rasa ayat ni satu ayat yang ‘inncocent’, biasanya keluar dari bibir anak-anak kecil yang masih dalam keadaan fitrah. Tapi hari ini saya sedar, betapa orang dewasa (saya), bila bertambah ilmu tentang dunia, semakin banyak bergantung kepada perkara keduniaan dan meminggirkan perhantungan kepada Allah. Dan ayat se‘innocent’ itu hanya mampu lahir dari jiwa yang benar-benar menyerah diri kepada Allah, di waktu susah dan senang.

Duhai sahabat, aku tak pernah berhenti mengagumimu sejak aku mula mengenalimu. Semoga Allah merahmatimu, putera dan puterimu serta ahli keluargamu, juga semoga rahmat Allah itu melimpahi teman-teman di sekelilingmu (termasukku).

Bismillah…

I was on night shift that day when the specialist and the MOs invited us for dinner after we finished our night round. The specialist, in particular, loves to talk, teaching us the basis of every disease we encounter during rounds, and telling stories outside medical field including politics. Indeed, I enjoy every moment when it is outside the hospital/medical-related thing. Sometimes we need a break to prevent exhaustion.

He suddenly asked us; why do you want to be a doctor? A very common question – from MO, specialists, head of department, director…. There are times they really are really curious, but most of the time they just made fun of anyone who become a doctor as their parents’ wish. That made most junior doctors, including me, very reluctant to answer.

So I just smiled, reserving my answer. Then my MO started to confess how he regrets his decision to become a doctor, and said it would be better if he now is a pilot or engineer. But well, despite his regrets, he is doing well in his carrier, and I would say he is one of the respected MOs in our department.

Every deed is counted based on our intention.

Should I tell the world that I become a doctor because my parents told me so? So that if I could not cope in this field I can simply blame them for my incompetency?  NO!

Indeed, it is not easy for everyone to love every single thing around them. There are times, or most of the time, I would say, we have to force ourselves to love what is in front of us. Even the 5 prayers, I could remember how hard it was for me to really do it sincerely and solemnly. Yes, it was difficult until I am really able to ‘blend’ every action (including solah, work, and other daily activities) as a part of my life in preparing the life in the hereafter.

‘Umar b. al-Khattab narrated that the Prophet (S) said: Deeds are [a result] only of the intentions [of the actor], and an individual is [rewarded] only according to that which he intends. Therefore, whosoever has emigrated for the sake of Allah and His messenger, then his emigration was for Allah and His messenger. Whosoever emigrated for the sake of worldly gain, or a woman [whom he desires] to marry, then his emigration is for the sake of that which [moved him] to emigrate.” [Volume 1, Book 1, Number 1]

I believe that every muslim is familiar with this verse – the first verse that imam al-Bukhari recorded in his book and so imam an-Nawawi, to point out that all deeds without proper intention are vain. Imam as-Syafi’e said that this verse comprises one third of all religious knowledge, and imam Ahmad said this is one of the 3 principles axes in islam in term of hadith. [Jawaami’ el-Kalim by Imam Ibn Rajab]

Alhamdulillah, I have everyone around to remind me regarding this important verse of Allah’s apostle. Every morning my mother would remind me to renew my intention, and that Allah has guaranteed abundance of rewards if I perform my job well for His sake. Thanks mak! Jazaakillah

It is indeed not easy. As someone who used to have some adjustment problem, I really need to fight to free my own self from selfishness to selflessness. I used to hate doctors and hospitals, but now I am a part of it. I could have chosen something I favour, but Allah’s favour is beyond everything. Everyday we pray asking him to show us he right path, and he has made this easy for me, so which of the favour of my Lord that I would deny? [Quran 5: 13, 15, 18 et al]

It may be that you detest something which is good for you; while perhaps you love something even though it is bad for you. God knows, while you do not know. [Quran 2: 216]

Stop whining

Previously since I was in medical school, we thought that studying medicine has taken so much of our time. We also had in mind that once we start to work we would be so busy that we would no longer have the life of a normal people. We would have deprived sleep, have no time to go for sightseeing or shopping, no time to attend classes or da’wah activity. But that is of course untrue.
But as the Malay sayings say, “Hendak seribu daya, tak hendak seribu dalih.” What ever we do and where ever we go, if we let the environment to control ourselves, we would not be able to survive. Being an engineer, or a teacher might be even busier than a doctor at certain times. If we are the type of person who always complain, then we would quit from everything we do. But if we just follow the flow and let ourselves to create a good working environment, we will make it someday.

I whined A LOT. Yes, I used to, and it changed nothing. Then I realised, I have to stop whining and act!

Now that I have worked for about a year as an intern, I still have time to go to Langkawi, went for bowling, baking our favourite cheesecakes, cooking, going for my weekly study-circle (usrah) and attending short Islamic courses. And I really SLEEP a lot!

Rescue in islam is obligatory (fard)

Narrated Abu Huraira: Allah’s Apostle said. “There are three persons whom Allah will neither talk to nor look at nor purify from (the sins) and they will have a painful punishment. (They are): (1) A man possessed superfluous water on a way and he withheld it from the travelers. (2) a man who gives a pledge of allegiance to a Muslim ruler and gives it only for worldly gains. If the ruler gives him what he wants he remains obedient to It otherwise he does not abide by it and (3) a man bargains with another man after the Asr prayer and the latter takes a false oath in the Name of Allah) claiming that he has been offered so much for the thing and the former (believes him and) buys it.” [Volume 3. Book 48. Number 838]

Rescuing or helping people in distress is an obligation in islam. And every muslim holds the responsibility to help people in need with regard to their knowledge. Doctors are like the man who possesses superfluous water – offering the medical care, while patients are like the travelers who come and go to seek treatment. Teachers teach who is seeking knowledge, engineers build the roads/buildings/computers… even a cleaner has their contribution to the public! (-of which we tend forget!)

Narrated Umar ibn al-Khattab: The Prophet (peace be upon him) said: the same occasion: Help the oppressed (sorrowful) and guide those who have lost their way. [Book 41, Number 4799: Sunan Abu-Dawud]

Narrated ‘Abdullah bin Umar: Allah’s Apostle said, “A Muslim is a brother of another Muslim, so he should not oppress him, nor should he hand him over to an oppressor. Whoever fulfilled the needs of his brother, Allah will fulfill his needs; whoever brought his (Muslim) brother out of a discomfort, Allah will bring him out of the discomforts of the Day of Resurrection, and whoever screened a Muslim, Allah will screen him on the Day of Resurrection.” [Book 43, Volume 3, Number 622: Sahih Buhari]

Therefore, we doctors, who hold at least a medical degree, need to fulfill our duties to the public/ummah. Providing the best healthcare is now an obligation (fard) for us.

Medical field is one of the doors for da’wah

I remember during my visit to Amman, Jordan few years back. Ustaz Razzi, who was doing his ‘specialty’ in tafseer, once told us – the medical/dental students – that being a doctor is a great opportunity for da’wah. How? I did not realise this until he said, people would come and look for us for advice.  Our patients ‘surrender’ themselves to us. And they would listen to every single words told by us. Unlike others (da’ie), according to him,  none will come to them other than people who really want to seek knowledge, and they themselves have to go into the society and call people to islam.

Aren’t we special?

But I guess this is the greatest challenge for me. It is easy to give medical advice to patients, but I would really need the courage and knowledge to call people to islam, not only to the patients, but among the healthcare workers as well as patients’ relatives. Yes, this is the best practical ground for US to practice all the theories in ‘enjoining what is right and forbidding what is wrong.’ [Quran 3: 104, 110, 114]

So dear Dr L, I would say I have no answer to your question ‘why would I want to be a doctor’, but I have the answer if you asked me ‘why I am a doctor.’

And I am here trying my best to fulfill my duty to Allah and my responsibility to the public by providing the best service as I could in the medical field and other field that I am capable of. For He has chosen US to be in this path, as a mean to ‘enjoin what is right and forbid what is wrong.’

Kalau dulu-dulu (sekarang pun), sebelum majlis kahwin, masing-masing sibuk mengumpul duit dan bahan untuk menghasilkan sebuah majlis yang sempurna. Jadi, begitu juga halnya dengan kehamilan, ia perlu dirancang sebaik-baiknya.

Kenapa perlu merancang kehamilan?

Ibu-ibu yang merancang untuk hamil perlu mula mengambil asid folik (folic acid/folate) sekurang-kurangnya 12 minggu sebelum hamil untuk mengelakkan penyakit yang disebabkan oleh neural tube defect. Kurangkan atau tinggalkan makanan berkafein, berhenti merokok, dan ambillah makanan yang berzat. Manakala ibu-ibu yang mempunyai masalah kesihatan seperti kencing manis atau darah tinggi, perlu mendapatkan nasihat doktor sama ada mereka selamat untuk hamil atau tidak. Perlu diingatkan, kehamilan boleh menjadi punca kemerosotan tahap kesihatan bagi ibu-ibu yang sudah menghidapi penyakit sebelum hamil. Dan kemerosotan itu boleh mempengaruhi kesihatan janin. Oleh sebab itu, amat penting untuk memastikan penyakit itu berada dalam keadaan terkawal sebelum hamil. Pastikan juga berat badan adalah optimum – tidak terlalu berat dan tidak pula terlalu ringan.

Maka hendaklah manusia memperhatikan makanannya. Sesungguhnya Kami benar-benar telah mencurahkan air (dari langit), kemudian Kami belah bumi dengan sebaik-baiknya. Lalu Kami tumbuhkan biji-bijian di bumi itu, anggur dan sayur-sayuran, zaitun dan pohon kurma,kebun-kebun (yang) lebat, dan buah-buahan serta rumput-rumputan, untuk kesenangan kamu dan untuk binatang ternakmu (QS ‘Abasa [80]: 24-32).

Persediaan mental juga amat penting bagi bakal-bakal ibu memandangkan tempoh kehamilan bukan untuk sehari atau seminggu-dua, tetapi 9 bulan 10 hari! Dalam tempoh ini, pelbagai cabaran yang perlu ditempuhi oleh si ibu yang pastinya memerlukan kekuatan minda dan emosi untuk menangani stress ketika mengandung, bersalin dan penjagaan anak selepas bersalin.

Jika kita merancang untuk hamil, pastikan juga kita merekodkan kitaran haid – lebih mudah untuk menjangkakan bila tempoh hamil itu bermula (bagi wanita yang mempunyai kitaran haid tetap). Dan sekiranya gagal untuk hamil, pelbagai langkah rawatan boleh diambil berpandukan paten kitaran haid masing-masing.

Banyakkan bacaan berkenaan kehamilan dan parenting. Jangan tunggu hamil atau bersalin baru mahu belajar!

Setelah hamil

Rancang penjagaan kandungan dengan menghadiri pemeriksaan ante-natal pada awal kehamilan dan ultrasound selweat-lewatnya pada minggu ke 20. Pemeriksaan ultrasound sebelum minggu ke-20 penting untuk menjadi kayu-ukur perkembangan janin seterusnya sehingga bersalin. Ikut nasihat doktor dan hadiri pemeriksaan mengikut tarikh yang ditetapkan.

Jika berkesempatan, hadiri kelas ante-natal yang dianjurkan oleh klinik/hospital kerajaan dan swasta. Hal ini sangat penting bagi bakal ibu dengan kandungan pertama khususnya bagi mendapatkan pendedahan tentang ilmu kehamilan dan kelahiran: perubahan fisiologi wanita ketika hamil, nasihat berkenaan pemakanan, pengenalan proses bersalin dan bagaimana menghadapinya, kaedah senaman ketika hamil dan selepas bersalin, teknik kawalan pernafasan (penting sewaktu bersalin atau hampir bersalin), kaedah kawalan kesakitan sewaktu hampir bersalin, kaedah penyusuan dan penjagaan bayi selepas lahir termasuk immunisasi dll.

Dengan menghadiri kelas ante-natal ini, para ibu akan lebih bersedia untuk menghadapi proses kehamilan dan kelahiran, mampu untuk memahami dan membezakan perubahan normal dan tidak normal pada diri mereka, dan tahu bila perlu mendapatkan bantuan/nasihat doktor. Ini adalah sebahagian usaha kita untuk memastikan keadaan janin baik dan bayi sihat. (Namun, harus kita mengimani bahawa setiap ketentuan itu adalah daripada Allah – walaupun sesuatu yang berlaku itu di luar dugaan kita)

“O mankind! If you are in doubt about the Resurrection, then verily! We have created you (i.e. Adam) from dust, then from a Nutfah (mixed drops of male and female sexual discharge i.e. offspring of Adam), then from a clot (a piece of thick coagulated blood) then from a little lump of flesh, some formed and some unformed (miscarriage), that We may make (it) clear to you (i.e. to show you Our Power and Ability to do what We will). And We cause whom We will to remain in the wombs for an appointed term, then We bring you out as infants, then (give you growth) that you may reach your age of full strength. And among you there is he who dies (young), and among you there is he who is brought back to the miserable old age, so that he knows nothing after having known. And you see the earth barren, but when We send down water (rain) on it, it is stirred (to life), it swells and puts forth every lovely kind (of growth).” (Quran- Al-Hajj:5)

وَالَّذِينَ يَقُولُونَ رَبَّنَا هَبْ لَنَا مِنْ أَزْوَاجِنَا وَذُرِّيَّاتِنَا قُرَّةَ أَعْيُنٍ وَاجْعَلْنَا لِلْمُتَّقِينَ إِمَاماً {الفرقان: 74}
